University Rotations

Please contact your rotation coordinator from your college/university to gain insight on the requirements related to your rotation. You and your rotation coordinator will be responsible for obtaining a provider for rotations.  

Steps for Observing for University Rotations

  1. OrthoIndy must have an affiliation agreement on file and active with your college/university.  
  2. If an affiliation agreement has not been established, OrthoIndy will decide if an agreement will be generated. If an affiliation agreement has not been established, the OrthoIndy legal team will draft an affiliation agreement, and send it to your school’s personnel for review. It's the responsibility of the student to provide OrthoIndy with the contact name and email for staff responsible for reviewing affiliation agreements. This process could take some time to establish final agreements, so please allow plenty of time prior to your established rotation date.  
  3. To obtain a provider, please visit the physician directory, where you can search the physicians by specialty. 
  4. Once you have a few providers selected, call the provider's office. You might have to leave a message or speak to the provider's support staff. Once you have a provider that has accepted you, please reach out to HR@OrthoIndy.com to start the application process. You will receive an email from the Human Resources department with the application. Please fill out all sections of the online form and attach the required support documentation (HIPAA signed form, Observer Liability Release form, current TB, MMR documentation, flu (if in season, malpractice insurance, and photo ID).
  5. The TB test cannot be expired or expiring before the end of your observing experience. You must have the current flu shot (required November 1st to April 1st), MMR documentation (2 dose proof required), malpractice insurance coverage from your college/university and current state driver's license, state-issued ID or student ID.
  6. The Human Resources department will process your application. Once approved you will receive an email with your approval letter. Please make sure you bring the approval letter along with some form of identification with you during your experience at OrthoIndy. 
  7. It's the responsibility of the individual seeking to observe to reach out to the provider to find out more details regarding arrival times, locations, etc.
