What do I need to bring to my appointment?
For clinic and imaging appointments, please bring the following items with you to your next appointment:
- Photo ID
- Insurance card
- Co-pay
- List of medications and dosage (do not bring medication with you)
- X-rays, MRI and CT scans that may be applicable to your appointment
For physical therapy appointments and surgery, please bring the following items with you to your next appointment:
Failure to bring these requested items may result in your surgery being rescheduled.
A parent or legal guardian must accompany minors under the age of 18.
For their safety, children under 12 years who accompany a patient, who are having imaging done, must be supervised in all waiting areas.
Because your exact surgery time may change, we will call you the day before your scheduled procedure to let you know what time to arrive.

You do not need to make an appointment for an Urgent Care visit. Check in at the front desk and let them know you are here for an urgent care visit. Please bring the following to your visit:

OrthoIndy West in Brownsburg has moved. It is located off of 600 N (East 56th Street) and Ronald Reagan Parkway.
- From the south: Take I-465 North to I-74 West. Go to the Ronald Reagan Parkway exit. Head north on Ronald Reagan Parkway to County Road 600 North. Take a left on to 600 N (East 56th Street). The facility is on the right.
