Request Medical Records

Please select one of the following options to request your medical records:

Option 1: Viewing Your Records via the Patient Portal

To utilize the patient portal, please register or log in at 29985.portal.athenahealth.com.

Once logged in, you can view and manage the following:

  • Appointments
  • Billing Summary
  • Health Reminders
  • Messages
  • Test Results
  • Request your medical records

If you have previously accessed Athenahealth through another provider or portal, please use the same login credentials.

Option 2: Requesting Your Records for Personal Use via Datavant

Medical record requests are managed by Datavant. In accordance with legal requirements, we must obtain your permission before releasing any copies of your medical records. Click here to be directed to the Datavant website where you can submit your request. If you’ve already submitted a request and would like to check its status, please call 1.800.367.1500.

Option 3: Requests for Records for Continuing Care

To ensure the fastest and easiest processing—without incurring a fee—please have your outside provider (such as your primary care physician or another physician’s office) fax a request for your records to 317.802.2050. If faxing is not an option, you may visit our Northwest office, at 8450 Northwest Blvd., Indianapolis, IN 46278, to complete the Request for Medical Records and Authorization form. Please note, our Northwest office is the only OrthoIndy location where you can complete and submit the form.

Or, you can click here to download the Medical Records and Authorization form. Once complete, fax the form to 317.802.2050. Records will be faxed to the provider or may be picked up at our Northwest office.

When requesting records for continuing care, turnaround time may be up to 24 hours from receipt of the request. 

Please Note

An authorization form signed by the patient or their legal representative is required for any records release, except when the request comes directly from another physician.
