Health, Dental and Vision

Insurance is goes into effect the first of the month following 30 days of employment. Our carrier is UMR and we utilize the United Healthcare OptionsPlus Network. We offer a High Deductible Health Plan and a traditional PPO plan. We also offer a Flexible Savings Account and Health Savings Account which allows employees to contribute pretax dollars that can be used for medical, dental and vision expenses. Full-time (30 hrs/wk) employees are eligible for coverage. We offer two dental plans through HRI and a vision plan through EyeMed.

Dependent Care Reimbursement

Eligible employees may contribute pretax dollars to this account to pay dependent care expenses paid to a licensed provider.

Profit Sharing 401(k) Plan

Employees meeting the eligibility requirements can participate in the plan on the first day of the month, after 90 days of employment. Employees are eligible for the corporate match which is dollar-for-dollar up to 6 percent. An annual Profit Sharing contribution is granted to eligible employees the January or July following their one-year anniversary.

Paid Time Off

Employees begin accruing paid time off (PTO) on the first day of employment and may take PTO after 90 days of employment. Employees accrue 136 hours of PTO per year in addition to 6 paid holidays. After five years of employment, employees will accrue 176 hours of PTO per year.

Paid Volunteer Time

Employees receive up to 16 hours of paid volunteer time for company-initiated community projects, such as Habitat for Humanity and building wheelchair ramps for SAWs.

Tuition Reimbursement

Full-time employees are eligible for up to $2,500 in tuition reimbursement annually, and part-time employees receive up to $1,250 annually. Employees must meet certain requirements to qualify. The course of study must be at an accredited institution and be related to the business of the company.

Employee Assistance Program

EAP offers short-term counseling to employees and immediate family members who live in their household. Assistance includes: family, marital, and significant other relationships, grief and loss, coping with change, and referrals for legal and financial assistance. EAP services are provided at no cost to employees.

Life Insurance

OrthoIndy provides full-time employees with life insurance equal to one times their annual salary. This benefit is provided at no cost to employees.

Short-Term and Long-Term Disability

Full-time employees receive short-term and long-term disability coverage at no cost. Disability coverage provides income protection of 60 percent of their salary with a maximum benefit of $1,000/week for short-term and $6,000/month for long-term.

Part-Time Benefits

Part-time employees who work more than 20 hours per week are eligible for paid time off, prorated holiday pay and 401K.
